Ok Rena, I checked your accounts at all 3 casinos and here's the situation.
At Cocoa Casino, you're not locked out at all, but you do have 2 accounts, which is not allowed, so one had been closed. This is the likely the account that you've been trying to log in to. Usually when a player has 2 accounts one is closed and both are banned from bonuses. It looks like in this case though (since you didn't claim a promo at either) they just closed one of the accounts but the other is still bonus eligible. If you don't remember your login/pw to the one that is still open I'm sure support can help you with this.
At Paradise 8, your account was no longer eligible for bonuses likely because you signed up in 2009 and have claimed 3 No deposit
promotions but have never made a deposit. When this happens, the casino is set up to stop granting bonuses because it would appear that you are only interested in free money and have no intentions of depositing. This is not always the case (like in your case) but 99% of the time it is. It's a safety mechanism for the casino and I think it's justified. That said, I changed the account back to bonus eligible for you.
At TIV, you've never made a deposit but are still very much bonus eligible
So to recap, you are now bonus eligible at all 3 casinos, you'll just have to contact support to give you the proper login for CC since you're trying to sign in to the wrong account.
